The ‘Focus on the Person’ form is a communication tool developed to help people living with dementia who are admitted to hospital.
The ‘Focus on the Person’ form was developed in 2017 following a study involving teams from Curtin University and Sir Charles Gairdner Hospital, working alongside a reference group inclusive of family carers of people with dementia.
